  • CDL copy (valid, correct class, endorsements)
  • DOT medical certificate (current, not expired)
  • Employment application (FMCSA Form)
  • Motor Vehicle Record (MVR) — annual review required
  • Pre-employment drug test result
  • Road test certificate or equivalent
  • Previous employer safety performance history (3 years)

Vehicle Inspection Checklist
